PROGRAM PURPOSE

Allergy and Immunology

The purpose of these lectures is to provide attendees with an overview of selected allergy/immunology topics. After this series of discussions, they should be able to better manage patients who have these conditions. Many patients carry a diagnosis of drug allergy, yet many are not actually allergic. It helps to understand diagnostic and management approaches. Telemedicine is a relatively new technology that is increasing rapidly. The attendee will learn what this new technology has to offer and how it might be incorporated into their practice. Many providers miss the signs of primary immunodeficiency. The attendee will learn the signs of PID and how to make the diagnosis. Providers often fail to address environmental factors that contribute to allergies.

The attendee will learn about environmental assessments, how to interpret a report and which interventions work. Asthma is often underdiagnosed. There is also a tendency to not prescribe controller medications or to provide patients with an asthma action plan. The attended will learn to manage asthma according to the NHLBI guidelines. Providers have a difficult time identifying diagnosing patients who have food allergies. They don't know when to do tests for food allergy or how to interpret the results. The attendee will be able to diagnose food allergy and to prescribe a food allergy action plan. New methods for identifying the causes of rhinitis and its treatment are not well understood. Providers lack a basic understanding of when allergen immunotherapy should be used and how to manage patients who are receiving it. The attendee will diagnose allergic rhinitis and recognize when allergy shots are indicated. Providers hve become frustrated with the transition to EMRs. Yet there are efficient ways to use them and to comply with requirements for meaningful use. This presentation will describe these methods and the use of registries to report outcomes.

  1. Asthma
    • Diagnose asthma severity, prescribe appropriate treatment and develop an asthma action plan. Attendees will also be able to diagnose vocal cord dysfunction as part of the differential of asthma and prescribe appropriate management.
  2. Food Allergy/Anaphylaxis
    • Properly diagnose food allergy using appropriate tests. Diagnose and treat anaphylaxis when it occurs.
  3. Allergic Rhinitis/Allergy Shots
    • Identify the underlying causes of patients with rhinitis and prescribe appropriate medical management including nasal sprays
  4. Immunodeficiency/Recurrent Infections
    • Identify risk factors for primary immunodeficiency (PID)
    • Order and interpret tests for PID
  5. Environmental Control/Toxic Mold
    • Obtain a relevant environmental history and use it to advise patients how to reduce their exposure to allergens that are contributing to their illness.
  6. Urticaria/Angioedema
    • Perform a diagnostic workup for a patient with urticaria/angioedema Manage a patient with chronic urticaria
  7. Telemedicine in clinical practice
    • Describe different types of telemedicine
    • Select digital equipment for examining patients via telemedicine

Dr. Jay Portnoy is an allergist in the Division of Allergy, Asthma & Immunology and he is the Medical Director of Telemedicine at Children's Mercy-Kansas City and Professor of Pediatrics at the University of Missouri-Kansas City School of Medicine. He received his medical degree at the University of Missouri-Columbia School of Medicine and he did his pediatric residency at the Children's Mercy Hospital in Kansas City and his Allergy fellowship at the University of Michigan in Ann Arbor. Following that he returned to Children's Mercy Hospital.

Dr. Portnoy has published numerous articles in peer-reviewed journals involving asthma disease management, environmental control and mold allergy. More recently he has been involved in use of Telemedicine to deliver patient care and in evidence-based medicine. He founded the UMKC School of Medicine allergy program and directed it from 1997-2006.

Dr. Portnoy served as President of the American College of Allergy, Asthma & Immunology in 2008 and he currently serves on numerous committees both of the American College and the American Academy of Allergy, Asthma & Immunology. He also is Chair of MOC for the American Board of Allergy and Immunology and he serves on the FDA advisory panel for allergenic extracts.

Dr. Portnoy lives in Overland Park Kansas with his wife and 2 cats. Fortunately, neither are cat-allergic.

Dr. Hyman is an internist with more than 30 years of experience and head of HymanHealth, which is located on the Westside of Los Angeles, California. A nationally recognized disability expert, Dr. Hyman provides medical-legal evaluations spanning the continuum of internal medicine for a wide variety of legal and insurance organizations. He has evaluated hundreds of workers’ compensation patients around the country, from NFL players to everyday workers, providing credible and thorough diagnoses, depositions and documentation, often using the state-of-science technology found in his office.

Dr. Hyman had served for 30 years as an Associate Professor on the clinical faculty at the University of California - Los Angeles (UCLA). He pursued medical school, internship, residency and fellowship at UCLA. Dr. Hyman is a highly sought-after lecturer, including being the keynote speaker for annual State level meetings. He has authored various medical-legal and internal medicine topics. He has served as editor for the Internal Medicine section of the Medical Disability Advisor, Fifth and Seventh Editions. Much of his work can be found in prestigious peer-reviewed journals including the Annals of Internal Medicine, the Journal of the American Medical Association, the Archives of Neurology, the American Family Physician, and the Journal of Occupational Environmental Medicine. Dr. Hyman is a chapter author and editor for numerous American Medical Association (AMA) books including:

Southampton (London), England - "Sir, when a man is tired of London, he is tired of life; for there is in London all that life can afford." Samuel Johnson's famous words are as true today as when he spoke them. A visitor to London could fill a month with fascinating people, art, music, late-night dancing, fabulous shopping, an endless array of pubs and first rate meals.

Tourists should take in Victoria Station, the beauty of Westminster Abbey, Parliament, and the pomp of Buckingham Palace. And then, there are the "Elgin Marbles" at the British Museum and the Crown Jewels in the Tower of London. Or visit Sherlock Holmes' HQ on Baker Street (you know the address). After all that, make what might be the most important stop of all: grab a pint at one of the many corner pubs that's been hosting revelers since before America was a nation.

London is most certainly a place to "take in," not just "pass through." You won't be bored.

After your time in London, you'll transfer to Southampton to board your ship and start your cruise conference. Options for doing that from bus transport to the train (which is very near the cruise port in Southampton) to private taxi are all solid choices.

In Vigo, Galicia province's most populated city, El Sireno, a gilded sculpture depicting a half-man-half-sea-creature, towers over your head and the city's lovely gardens and well-preserved historical buildings.

Tenerife, Canary Islands - The largest of the Canary Archipelago, the island of Tenerife consists of many different regions - each with its own characteristics.
