PROGRAM PURPOSE

Objectives:
The primary goal of this course is to provide essential and focused updates within the broad scope of primary care practice. To accomplish this, material will be collected representative of both the primary and specialty care literature, with special emphasis on evidence-based methodology and, in the absence of definitive EBM, consensus statements and other best practices.

Topics:
Objectives for the course have been designed to reflect specific spheres of clinical knowledge which require periodic review based on our changing understanding of pathophysiology and therapeutics. At the conclusion of this course, participants should be able to:
  1. Management of common infectious diseases in the ambulatory setting
    • Identify and manage common infectious diseases seen in an ambulatory setting, including the use of diagnostic testing as indicated and the selection of appropriate antibiotic regimens;
  2. Update from the ACIP on recommended vaccines (for pediatric and adult patients, including geriatric populations)
    • Utilize the appropriate pediatric and adult vaccines as recommended by ACIP (Advisory Committee on Immunization Practices) in its schedule;
  3. Common office orthopedic and rheumatologic conditions
    • Recognize the diagnostic criteria for, and expected management of, common orthopedic and rheumatologic conditions seen in a primary care environment;
  4. Common skin conditions and their management
    • Identify common skin conditions seen in the general population (along with currently-recommended therapies), plus identify patients whose dermatologic findings are representative of a more systemic illness;
  5. Office emergencies and pre-hospital care, with a focus on cardiovascular, neurologic, and pulmonary illnesses
    • Recognize the signs of acute illness in the office requiring pre-hospital management and transfer to an alternate level of care (emphasis here on cardiac, neurologic, and pulmonary conditions);
  6. Current topics in psychiatric illness and behavioral health
    • Describe common behavioral conditions (including psychiatric illness) seen in primary care practice and generate a treatment plan, including the use of psychotropic medication, counseling, and the co-management of select patients with a mental health clinician as indicated;
  7. Updating pharmacotherapy, including co-management of patients under specialty care
    • Demonstrate an understanding of the drug armamentarium available to clinicians for a variety of diseases commonly seen in ambulatory practice, and be aware of the rationale and pharmacology of novel medication treatments being prescribed by specialists to primary care patient populations;
  8. Imaging in Primary Care: What do we need to know before ordering studies on patients?
    • Order appropriate imaging studies for patients within their practice population using ACR (American College of Radiology) criteria;
  9. Screening: What to do and when, and what not to do, according to the USPSTF
    • Know the current recommended screening practices of the USPSTF (United States Preventive Services Task Force);
  10. Promoting Wellness and Population Health: the role of the generalist
    • Articulate a concept of wellness and population health appropriate for primary care clinicians;
  11. What lab testing, and which procedures, are recommended for the primary care clinician's office?
    • Identify the testing procedures and equipment useful in an office/ambulatory setting;
  12. GYN and Pediatrics: What is an appropriate scope of practice in these disciplines for primary care practitioners?
    • Recognize which aspects of pediatric care and office GYN are appropriate and within a primary care practitioner’s scope of practice;
  13. The medical management of pregnant patients
    • Identify and manage common medical and surgical illnesses seen in pregnant patients;
  14. Case Studies in Primary Care (in addition to those presented, participants will be encouraged to submit their own cases for group discussion)
    • Present specific cases from their own clinical experience illustrating challenging diagnostic and treatment issues inherent in primary care practice;
  15. BONUS lecture: How to Update our Fund of Knowledge - A Personal Perspective on CME, the Medical Literature, and Keeping Up in Clinical Practice
    • Create a plan for updating their fund of knowledge and maintaining broad clinical competency within their scope of practice.

Faculty

Louis Verardo, MD

A native of Long-Island New York, Dr. Verardo is a 1978 graduate of the University of Bologna, Italy, and a 1981 graduate of the Family Practice Residency Program at Glen Cove Hospital on Long Island. He worked for several years in solo rural practice in Rhode Island, where he participated in and published original research in Lyme disease. He taught residents affiliated with Brown University’s School of Medicine, and he created a new elective rotation in Community Medicine for that institution. In the fall of 1985, he returned to Glen Cove and worked for 11 years as Associate Director of his former program.

In 1996, he took a new position as Medical Director of the Dolan Center, in Huntington, New York, a facility for the uninsured and underinsured in that community. In 2006, he was appointed Assistant Medical Director for Ambulatory Geriatrics at the Parker Jewish Institute’s Diagnostic and Treatment Center. In 2008, he returned to private practice in the South Shore area of Long Island, where he remained until joining the full-time faculty at Stony Brook medical school in late 2009. He then engaged in clinical practice, as well as teaching medical and dental students, and until July of 2016, he had also served as an Assistant Director of Stony Brook’s Family Medicine Residency Program. Although Dr. Verardo retired from active clinical practice on November 30, 2019, he remains a member of the faculty for the medical school course called Medicine in Contemporary Society, as well as serving as a facilitator for the Transition to Clinical Care course in 2020.

Throughout his career, Dr. Verardo has maintained an active teaching presence as a community faculty for several other institutions, including Long Island University (CW Post campus), Adelphi University, Molloy College, Touro College School of Health Sciences, and the former North Shore-Long IslandJewish Health System, now known as Northwell Health.

In addition to his work in Lyme disease, he has published work in familial alcoholism and on maintaining skills in neonatal resuscitation, and he has participated in practice-based research with a variety of national groups, including the Ambulatory Sentinel Practice Network (ASPN). He is involved with the Suffolk chapter of the New York State Academy of Family Physicians (NYSAFP), serving as a county delegate, and he has reviewed program content for several organizations, including the Society of Teachers of Family Medicine (STFM), the Society of General Internal Medicine (SGIM), the American Geriatrics Society (AGS), and the Primary Care Network (PCN), a national CME program for generalist clinicians based in Springfield, Missouri .

Miami, Florida - As far as vacation destinations go, Miami tops most sun-seekers’ lists. With its gorgeous beaches, vibrant neighborhoods and melting pot of cultures, the Magic City is the perfect place to spend a few days before your Caribbean island hop. Ahead of your cruise from Miami, jump-start your morning in true Miami style with a Cuban cafecito from a ventanita — a streetside shop window. Then climb the historic lighthouse at Bill Baggs Cape Florida State Park on quiet Key Biscayne before lounging on the island’s secluded sandy shores. If you’re looking for a bit more action, head to South Beach — home to world-class restaurants, nightlife, shopping and lively beaches, all set against snap-worthy Art Deco buildings. Art lovers who visit Miami can head to Wynwood and explore the neighborhood’s colorful street murals and local galleries. Or take a trip back in time at the Renaissance-style Vizcaya estate, which offers a glimpse into Miami’s dazzling past.

At Sea - Cruising - Cruising

Philipsburg, St. Maarten - The capital of the Dutch side of St. Maarten is the town of Philipsburg, though St. Maarten is an island with two distinct sides. To the north is the French side, whose capital is Marigot, while Philipsburg sits on the southern part of the island. When exploring on cruises that go to St. Maarten, it’s important to visit both sides to understand their customs and heritage.

You’ll quickly be charmed by all sides of the island. Of course, this Caribbean island is best known for its beaches, like Maho Beach or Great Bay Beach, one of the area’s best snorkeling spots. Sample local seafood or rum during your time in port, which have become symbols of St. Maarten life. There’s just enough to fill the day here, especially if you hop between the French and Dutch beaches.

Charlotte Amalie, St. Thomas, U.S.V.I. - Named after a Danish Queen, Charlotte Amalie has one of the most breathtaking deepwater harbors in the world. The key sights of Charlotte Amalie are located within the town centre, making it easy to combine sightseeing with St. Thomas’ fantastic shopping opportunities. Eighteenth century architecture provides the perfect backdrop for walking tours and shopping here in the "Duty Free Capital of the World." For a dose of history, visit Emancipation Park, the American-Caribbean History Museum, or Bluebeard the pirate's castle at Magen’s Bay. If tropical bliss is what you're after, head to any of Charlotte Amalie’s pristine beaches, where the warm water, white sands and palm trees will lull you into deep relaxation!

Coco Cay, Bahamas - Located in the middle of the Caribbean's bright blue water lies the secluded island of CocoCay. With its white-sand beaches and spectacular surroundings, CocoCay is a wonderland of adventure.
