PROGRAM PURPOSE

The purpose of these lectures is to update clinicians/prescribers on pharmacological principles and practices that can be applied during Critical Care and Pulmonology interventions.

The desired outcome is that attendees will adopt the stated learning objectives to improve their professional practice.

A series of Pulmonary Diseases from COPD and Asthma to Sarcoid and Immunodeficiency will be presented. There are new guidelines for both Asthma and COPD. Sarcoid and Immunodeficiency are not uncommon but are missed frequently.

The Critical Care lectures will be on Community-acquired Pneumonia which is one of the most common infections in humans. New guidelines came out late last year. Attendees will be able to treat these using the most up to date guidelines. Ethics will be taught in the manner of case reports. Both on the treatment of patients, their families and our colleagues. Ventilator Associated Pneumonia also has quite recent guidelines. Attendees will know how to prevent, treat and for how long to treat the pneumonia. The controversies surrounding Vitamin C and steroids in Septic Shock and Fluid resuscitation in Septic Shock will be discussed. Attendees will have a better idea of these.

    • Be up to date with the new 2020 guidelines in the diagnosis and treatment of COPD
  7. Sarcoid
    • Attendees will have a new approach to the diagnosis, Treatment of Sarcoid and know the Multiple drugs for Sarcoid
  8. Immunodeficiency (ID)
    • Attendees will be able to treat Primary ID, Common ID and Variable ID. Learn diagnosis, treatment and asses cases.
  9. Ventilator Associated Pneumonia (VAP)
    • Attendees will be able to prevent , diagnose and treat VAP according to new guidelines
  10. Ethics
    • The Attendees will hear cases and I will ask their opinions. They may also present their own cases.
  11. Vit C, Steroids Fluid Resuscitation in Septic Shock.
    • Attendees will be aware of Septic Shock controversies surrounding the use of Vit C and Steroids and fluid resuscitation
  12. Community-acquiredPneumonia (CAP)
    • Attendees will learn to diagnose, treat according to new guidelines. Also who to admit to hospital or even the ICU.

Mark Rumbak, MD

Dr. Rumbak is a Professor of Medicine, Division of Pulmonary, Critical Care & Occupational Medicine in the Department of Internal Medicine, at the University of South Florida College of Medicine, Tampa, Florida. He is the Director of Bronchoscopy & Chief in the Clinical Research Unit, Pulmonary & Critical Care at James A. Haley Veterans' Hospital, Tampa, Florida. 

Dr. Rumbak is from Johannesburg, South Africa, where he went to medical school and did his residency and Fellowship in Pulmonary and Critical care medicine. He came to the US IN 1987, spent five years at the University of Tennessee in Memphis, and then came to the University of South Florida in Tampa. Dr Rumbak's main interest is interventional pulmonary medicine, including pulmonary arterial hypertension. He is the Governor for Florida for the American College of Chest Physicians. He is a full professor and is on staff at four hospitals including the James A Haley Hospital, Tampa General Hospital and Florida Hospital, Tampa.

 Tammie Lee Demler received her Doctor of Pharmacy, Master of Business Administration, and Bachelor of Science degrees at the State University of New York (SUNY) at Buffalo. 

Dr  Demler is a dually Board-Certified Specialist in both Psychiatric and Geriatric Pharmacy and is currently the Director of Pharmacy Services and Pharmacy residency training at the New York State Office of Mental Health (OMH) at the Buffalo Psychiatric Center. 

She holds numerous academic appointments, including Clinical Associate Professor for SUNY Buffalo School of Pharmacy and Pharmaceutical Sciences, Clinical Assistant Professor at SUNY School of Medicine Department of Psychiatry, D'Youville College School of Pharmacy, University of Florida School of Pharmacy, and Clinical Instructor for the Erie County Community College Pharmacy Technician Certification Program.

She  collaborates with both the National Association of the Boards of Pharmacy (NABP) and the Board of Pharmaceutical Specialties (BPS) to develop board examinations that establish the minimum competencies expected for new practitioners for practice in an ever changing clinical environment. Prior to embarking on her academic and institutional career, Dr. Demler's clinical practice focused on ambulatory care in community pharmacy.

Dr. Demler has authored and co-authored numerous original research articles and expert reviews for print media. She is a regular contributor to U.S. Pharmacist and reviewer for Pharmacist's Letter

Prior to launching Pharmacotherapeutics for Advanced Nursing Practice, Dr. Demler authored a chapter on insomnia in the American College of Clinical Pharmacy's (ACCP's) Ambulatory Care Self-Assessment. She also has provided expert pharmacotherapeutic consultation for other books such as the Side Effects of Drugs: Annual Volume 36 and Clinical Consult to Psychiatric Nursing for Advanced Practice . She has presented medication continuing education programs internationally and nationally, and is an Academic Educator for the New York State Medicaid Prescriber Education program sponsored by the New York Department of Health. 

Dr. Demler served as President of both the Pharmacists Society of the State of New York and the Pharmacists Association of Western New York. Dr. Demler is also a local media personality, hosting her own weekly TV talk show as well as a weekly community affairs radio segment. She has won a number of prestigious national, state, and local awards acknowledging both her professional and community-based contributions.

The College of Family Physicians of Canada (CFPC) has a reciprocal credit reporting agreement with the American Academy of Family Physicians (AAFP). Members of the CFPC may claim up to 50 credits per five-year cycle from Physician's Recognition Award (PRA) Category 1 credits. Any additional credits are eligible as non-certified credits.

Date Description
Day 1 Dublin-Kilkenny-Cashel-Killarney: After a leisurely breakfast we meet our Driver Guide in the lobby of the hotel. We drive through Dublin and head south. We continue to the Medieval City of Kilkenny, where we enjoy an optional visit The Kilkenny Castle which has been completely refurbished in recent years.

It is then onto the Rock of Cashel, where we may take an hour tour of the Cashel Cloisters. Our next stop, Blarney Castle, is home to the blarney stone, which legend has it bequeaths the "gift of eloquence" to all who kisses it.

Time to shop at the woolen mills or stop in Christy's Pub for a Pint! Our journey takes us east over the Kerry and Cork Mountains to Macroom where we pause for a short visit. This is the main market town for the Gaelic speaking region to the west. Macroom Castle dates from the 13th century and was the seat of the McCarthy's of Muskerry. Oliver Cromwell granted it to Admiral Sir William Penn whose namesake son spent much of his childhood here and later founded the Commonwealth of Pennsylvania. We arrive Killarney. Time permitting, we may visit Victoria Muckross House. Overnight at The Great Southern Hotel .
Day 2 Dingle: After breakfast we depart Killarney for the Dingle Peninsula. The journey begins with some of the most magnificent scenery in all of Ireland. The little town of Dingle is straight out of a story book. The fishing boats in the harbor and tiny brightly painted pubs portray a time that has continued to stand still. The population of 1500 people and only 52 pubs are ready to welcome you this afternoon. Worth the wait is the native seafood chowder served up at lunch time with freshly baked brown bread. We continue on to Slea Head. Along the way, we will see beehive style huts, originally inhabited by 12th century monks, which overlook the rugged Blasket Islands. The awe-inspiring beauty of this particular area is unmatched.

There is no other landscape in western Europe with the density and variety of archaeological monuments as found on the Dingle Peninsula. OR Ring of Kerry. After breakfast we enjoy an optional tour to The beautiful Gap of Dunloe in the Killarney National Park. We then start on a full day tour of "The Ring": There is an unspoiled nature to Ireland's most beautiful region and the Ring of Kerry provides many unforgettable memories as it passes through the many picturesque villages such as Glenbeigh, Waterville and Sneem and returns via Ladies View and the famous Lakes of Killarney. We continue to Kenmare and Molls Gap where we visit (optional) the Kissane Sheep Farm. This traditional Farm has approximately 1000 sheep. Time permitting you might also enjoy an hour of Horseback Riding at the Killarney Stables. Be sure and visit one of the three Lakes of Killarney, Lough Leane, The Upper Lake and The Lower Lake which is near the town. OvernightThe Great Southern Hotell.
Day 3 Killarney–Kinsale-Blarney-Cork : We continue our back roads journey. We travel down through Glengarriff past Bantry Bay and Bantry, where we stop for a visit. Bantry sits at the head of one of Ireland's most beautiful bays. Its narrow streets lined with shops have changed very little over the last 100 years. We continue on to Skibbereen, made famous in the Johnny Cash song "Forty Shades of Green". Next, we visit the fishing village of Baltimore, where we stop for lunch (on own) and take in the beautiful views of the Atlantic Ocean. After our stop, we journey along the southern coast of Ireland, with its unmatched back road scenery, to Kinsale. Known as the Gourmet Capital of Ireland, Kinsale is truly a culinary delight set in a postcard fishing village. We continue to Cobh for an optional stop at the Cobh Heritage Center depicting emigration from Ireland during the great famine of the id 1800's. It is then onto Cork for our overnight. Day 4 Cork-Dublin: Transfer from Cork to Dublin Port for arrival by noon.
